[pox-dev] Switch & Host Disconnect

There's a ConnectionDown event when switches disconnect.  For hosts, it's possible you'll see this reflected in port status.  Also, the host_tracker component tries to track the state of hosts.  But in the case of hosts, it's all really just inference -- the only entities OpenFlow is really concerned with are switches.
